Adaptation to better systems as well as up gradation to technical advancement is one thing I believe in incorporating into patient care at our various centers. A few recollections from the Article 'Gasless Laparoscopy attempted in the City', in the INDIAN EXPRESS, dated 17TH March 1998. The first gasless laparoscopy surgery in South was performed on a 25- year old woman by city-based surgical Gastroenterologist J.S.Rajkumar. During the Surgery, done under regional anesthesia the patient's appendix as well as a cyst on her ovary were removed. She was discharged in 48 hours. While gasless laparoscopy is done by some Delhi surgeons, this is the first time this procedure has been done in the South. But, normally, laparoscopic procedure involves using carbon dioxide to bloat the abdomen, after which the tubes are put in to remove the particular organ. "We pump in two to three liters of carbon dioxide and there is risk of the gas getting absorbed by the patient's body, resulting in harmful side effects in the elderly or those with lung problems. More important than this, the bloating of the abdomen pushes up the diaphragm towards the chest which could interfere with the respiratory function of the patient", says Dr J S Raj kumar. Gasless laparoscopy is done by using modified crane, and almost all cases it obliterates the need for giving general anaesthesia. Recollecting my first gasless laparoscopy surgery performed in 1998 , I can say the institution has always strived to incorporate the latest technical development with possible innovation into the treatment offered.
